There are two ways to submit your Waiver Form -
- You can complete the Form below and click Submit. YVDC will receive your entry electronically, you will also receive a copy to your email address.
- Alternatively, you can click the Print button and print out a hardcopy of entry form and either
- scan and submit it via email or
- post to Yarra Valley Dressage Club, PO Box 297, Wandin, Vic. 3139
